ODMHA has requested additional insurance coverage be requested for all off-ice events. Insurance coverage for dry land training, team building, or team parties will all need to be applied for. The Special Events request for Insurance form must be submitted prior to the event to ensure coverage. Visit the forms webpage to find link for the online form complettion and submission.
